| Tribal Application
Please reade all instructions on the first page of the Application and complete all fields that reqire data. You are also required to submit a copy of your Birth Certificate and a Family Tree. Also ensure you get the Notary page signed and stamped by a license Notary, each individual applicant must sign this page unless they are under age, then a guardian can sign.
Please contact Chief Marshal (Lone Wolf) Couch as instructed on the first page prior to submitting your application. This is for a preliminary review.
The Application and Family Tree documents are located at the bottom of this section as PDF Files. By clicking on each one they will open the file and allow you to print them.
